Church & Contact

Pastor, church, denomination, insurance contact, addresses, effective date, year founded, and organizational details.

Property Schedule

Occupancy, addresses, protection class, building and contents limits, valuation basis, deductibles, construction, area, stories, year built, roof, systems, and fire protection.

Coverage Requests

Property extensions, liability limits, D&O, employment practices, sexual misconduct, cyber, religious expression, and other requested protection.

Ministry Operations

Attendance, employees, payment plan, schools, daycare, camps, cemeteries, nonprofits, safety programs, cooking, events, athletics, energy sources, and vehicles.

Risk Controls

Building conditions, historical status, vacancy, abuse-prevention policies, background checks, supervision, crisis planning, and reference checks.

History & Authorization

D&O and EPL history, additional interests, five-year claims, current carrier and premium, nonrenewal history, signature, title, and referral source.

What should accompany the completed RFP?

Missing documentation can delay review or prevent an underwriter from offering terms. Additional information may be required based on the ministry, property, operations, or market.

Current declarations

Complete declaration pages for every applicable policy and line of coverage.

Five-year loss runs

Currently valued carrier loss reports covering the requested five-year period.

Building photographs

Exterior, interior, roof where safely available, systems, and special property features.

ACORD applications

Completed applications for commercial auto, workers’ compensation, and umbrella when those coverages are requested.

Sensitive information and documents

Do not send the full RFP through the preliminary website form.

The complete RFP may include FEIN, loss history, policy documents, addresses, building data, signatures, and other nonpublic information. After the preliminary request is received, the 1225 Unified team will provide the approved RFP and instructions for returning required documents through the appropriate submission method.
