If you continue beyond a preliminary inquiry, additional information may be required through an approved submission method, including property details, financial and payroll information, loss history, policy documents, photographs, FEIN, signatures, drivers, employees, and underwriting information. The public website forms intentionally do not request highly sensitive documents or identifiers.
